Job summary

Molina Healthcare is seeking an experienced Registered Nurse to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate integrated member care, develop and monitor care plans, and support members toward desired outcomes.

The role requires at least 2 years in care management or LTSS, an active RN license, and the ability to travel regionally. Strong communication and proactive problem-solving are essential to succeed in this position.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care, with care management or LTSS exposure.
• Experience with disability/chronic conditions and behavioral health settings is preferred.
• Proactive, detail-oriented with strong communication skills.
• Valid driver license and ability to travel 25–40% as needed.

Responsibilities

• Conduct comprehensive member assessments within timelines and may perform in-person home visits.
• Facilitate waiver enrollment/disenrollment and coordinate care plans with stakeholders.
• Monitor care plans, document outcomes, and adjust as needed to meet member goals.
• Coordinate LTSS and behavioral health services across the care continuum.
• Lead or participate in ICT meetings for service approvals and care collaboration.
