Job summary

Molina Healthcare is seeking a Registered Nurse with care management experience to support LTSS and waiver services. Qualifications

• RN licensure active and unrestricted.
• 2+ years healthcare experience including care management.
• Experience with persons with disabilities, chronic conditions, LTSS, or related fields.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care networks.
• Monitor and evaluate care plan effectiveness and adjust as needed.
• Coordinate services across LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services; review benefits and funding sources.
• Lead or participate in interdisciplinary care team meetings.
• Travel 25–40% as required by state/contractual rules.
