Job summary

Responsibilities include completing assessments within timelines, coordinating waiver enrollment, developing care plans with providers and family, monitoring plan effectiveness, and assisting with barriers to care.

Qualifications

• At least 2 years of experience in health care, with 1 year in care management or related setting.
• RN license active and unrestricted in state of practice.
• Bachelor's degree in health care field may be required in some states.
• Valid driver’s license and reliable transportation for travel requirements.
• Ability to work independently with minimal supervision and strong communication skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and others.
• Monitor care plans for effectiveness and document outcomes.
• Promote integration of services across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Support ICT meetings for approvals/denials and collaboration.
• Provide education and motivational interviewing to members.
• Identify barriers and coordinate resources to address psycho/social, financial, and medical needs.
• Travel 25-40% as required by state/contractual requirements.
