Job summary

SnapCare is seeking a Registered Nurse for the Day shift in Des Moines, IA. The role focuses on rapid bedside assessments, developing individualized care plans, and documenting patient progress within the medical record.

You will participate in rehabilitation activities and support discharge planning from admission. The position requires at least two years of bedside experience and recent critical care exposure, with BLS and ACLS certifications required before the first shift.

Qualifications

• Minimum two (2) years of bedside clinical experience.
• Recent critical care experience.
• BLS Certification required prior to first shift.
• ACLS required prior to first shift (CIRH only).

Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission.
• Develop and maintain individualized plans of care based on assessment data.
• Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences.
• Document all assessments, interventions, and patient responses accurately and timely in the medical record.
• Participate in patient rehabilitation program on assigned unit.
• Initiate discharge planning from day of admission and provide patient teaching in anticipation of discharge.
• Communicate pertinent information regarding changes in patient status to attending physician and Charge Nurse.
• Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and/orientees.
• Assume charge responsibility following orientation period as requested.
• Maintain safe patient handling practices and adhere to all safety policies and procedures.
