Waverly Rehabilitation And Healthcare Center in Petersburg, VA, is hiring a Wound Care Nurse (F/T). You will develop and execute individualized wound care plans with an interdisciplinary team, aiming to preserve skin integrity and improve patient outcomes.

The role requires an active RN or LPN license and LTC/SNF experience is preferred. Excellent collaboration with medical staff and adherence to regulatory standards are essential to success in this position.

• Develop and ensure the wound care plan in collaboration with an interdisciplinary team.
• Preserve skin integrity by implementing skin preservation strategies.
• Ensure all wound care protocols are approved by medical staff.
• Participate in weekly multidisciplinary wound and skin rounds for high risk patients and significant wounds.
• Provide hands-on wound care for difficult/non-healing wounds.
• Organize and implement the restorative program.
• Ensure compliance with regulatory requirements.

Summary/Objective:

In this role, you will develop and ensure the wound care plan specific for each patient in collaboration with an interdisciplinary team.

• Preserving the integrity of the skin by implementing and monitoring skin preservation strategies.
• Ensuring all wound and skin care protocols are approved by the medical staff.
• Participatingmultidisciplinarywound and skin rounds on a weekly basis for high risk patients and patients with current pressure ulcers and other significant wounds.
• Providing hands-on wound care for all difficult and non-healing wounds.
• Organize and implement the restorative program.
• Ensuring all regulatory requirements are met.
