Supreme Touch Home Health Services, Corp. is seeking a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home health team serving the Columbus area.

This visiting role involves approximately 20 patient visits per week, providing compassionate, patient-centered care in the home setting. Ideal candidates have at least two years of home health experience, strong communication skills, and knowledge of state and federal nursing regulations.

Formación

• Two or more years of experience in home health nursing.
• Knowledge of state and federal regulations.
• Excellent verbal and written communication skills.
• Ability to assess and prioritize patient needs using sensory and cognitive skills.
• Ability to work independently and collaboratively with healthcare teams.
• Experience with multidisciplinary teams and coordinating care.
• Ability to lift and move patients/equipment as required (over 50 pounds).

Responsabilidades

• Adhere to the scope of practice per State Board of Nursing.
• Preserve patient dignity and privacy.
• Maintain patient confidentiality.
• Administer prescribed medications within scope.
• Collaborate with other health care providers and educate patients/significant others.
• Document and report care, changes, and clinical observations to RN Clinical Manager.
• Perform additional duties as requested.

IN NEED OF A VISITING NURSE RN OR LPN - APPROXIMATE 20 VISITS PER WEEK- SOUTHSIDE COLUMBUS.

Qualifications

• Minimum of two (2) years of experience as a Professional Nurse in home health care setting
• Knowledge of State and Federal regulations
• Proficient oral and written communication skills
• Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
• Ability to independently seek out resources and work collaboratively
• Ability to work with a variety of groups; multidisciplinary teams, managers, and staff
• Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/ or equipment over 50 pounds

Duties and Responsibilities

• Adhering to the scope of practice as set forth by the State Board of Nursing
• Preserving the dignity and self-respect of patients
• Practicing patient privacy and confidentiality
• Administering prescribed medication within the state scope of practice
• Collaborating with other health care providers and provide education to patients and/ or significant others
• Documenting and reporting care received, changes, observations and other data related to the clinical condition of the patient to the RN Clinical Manager
• Performing other duties as requested
