Job summary

BrightStar Care of East & NW Columbus is actively seeking an LPN Homecare Visit Nurse to provide compassionate nursing care during home visits across the Columbus area. This full-time role offers the opportunity to improve patients’ health in familiar home settings while supporting families and caregivers.

The position requires an Ohio LPN license with at least 1 year of nursing experience, preferably in home health.

Qualifications

• Current LPN license in Ohio.
• Minimum of 1 year of nursing experience, preferably in a homecare or similar setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a collaborative team.
• Valid driver's license and reliable transportation.
• One year of experience as a home care professional nurse and is competent in performing home care comprehensive assessment.

Responsibilities

• Conduct home visits to assess patients' health status and provide nursing care as prescribed by the healthcare team.
• Administer medications, treatments, and procedures according to established protocols and physician orders.
• Monitor patients' vital signs, symptoms, and progress, and report any changes or concerns to the appropriate healthcare professionals.
• Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health outcomes.
• Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to develop and implement individualized care plans.
• Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record.
• HHA Supervisory visits to monitor care.
• Wound care
