Job summary

Supreme Touch Home Health Services, Corp. is seeking caring Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home health team in the Columbus area, with approximately 20 visits per week on the Southside of Columbus.

We value compassionate, patient-centered care and strict adherence to state and federal regulations. Responsibilities include medication administration, patient education, and coordinating with the care team to ensure quality, safe home health services.

Qualifications

• Minimum of two (2) years of experience as a Professional Nurse in home health care setting.
• Knowledge of State and Federal regulations
• Proficient oral and written communication skills
• Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up

Responsibilities

• Adhering to the scope of practice as set forth by the State Board of Nursing
• Preserving the dignity and self-respect of patients
• Practicing patient privacy and confidentiality
• Administering prescribed medication within the state scope of practice
• Collaborating with other health care providers and provide education to patients and/ or significant others
• Documenting and reporting care received, changes, observations and other data related to the clinical condition of the patient to the RN Clinical Manager
• Performing other duties as requested

Qualifications

• Minimum of two (2) years of experience as a Professional Nurse in home health care setting

• Ability to independently seek out resources and work collaboratively
• Ability to work with a variety of groups; multidisciplinary teams, managers, and staff
• Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/ or equipment over 50 pounds
