Lifestyle Medicine in Sheboygan, WI seeks an RN or LPN Supervisor to support resident care, oversee daily operations, and lead Direct Care Professionals across two assisted-living homes.

You will balance hands-on nursing oversight with team leadership, coordinate care plans, manage medication processes, and ensure regulatory compliance. The role emphasizes strong communication, training, and reliable on-site presence, including weekend coverage on a rotating basis.

Formación

• Active RN or LPN license required.
• AFH/CBRF Administrator course completion preferred.
• Experience in assisted living or long‑term care preferred.
• Ability to coordinate care plans and staff training.

Responsabilidades

• Oversee resident care, update care plans, and communicate changes.
• Coordinate medical appointments, referrals, and transportation.
• Oversee medication systems, MAR/eMAR review, and inventory.
• Lead DCP onboarding, training, and competency checks.
• Manage scheduling, timecard review, and staffing coverage.
• Maintain safe food storage, meal readiness, and menus.
• Ensure cleanliness, safety, and daily home routines across two homes.
• Maintain documentation accuracy and compliance records.
• Serve as on-site supervisor weekdays and every fourth weekend; hand off to leadership.
• Communicate with residents, guardians, staff, providers, and Executive Director.
