Job summary

Avantara seeks a motivated Hospital Liaison based in Rapid City, SD, to oversee regional business development and drive census growth for Avantara locations. The role focuses on building partnerships with hospital discharge planners, creating community connections, and ensuring smooth transitions for residents across the region.

The ideal candidate has a background as an LPN, RN, or Social Worker with 2 years in the long-term care industry and will travel regionally to support multiple

Qualifications

• Current, unencumbered LPN or RN license in South Dakota or related patient care coordination background.
• Experience in business development, hospital liaison, or healthcare sales with a focus on relationships and strategic planning.
• In-depth knowledge of post-acute care, long-term care, or related healthcare industries.
• Outstanding communication and interpersonal skills with knack for building lasting partnerships.
• Ability to travel regionally to support multiple Avantara facilities and foster community partnerships.

Responsibilities

• Cultivate and maintain relationships with referral sources, including hospital discharge planners, case managers, and social workers across your region.
• Promote Avantara’s post-acute care services to healthcare providers, hospitals, and community organizations.
• Collaborate with facility leadership to optimize admissions processes and enhance resident and family satisfaction.
• Provide clinical onsite coordination with local hospital networks to ensure continuity of care during transitions.
• Develop and execute strategic plans to increase census growth based on data-driven insights and market trends.
• Represent Avantara at networking events, community engagements, and healthcare expos to strengthen brand presence.
• Monitor referral pipelines and provide regular reports on activity, conversions, and growth metrics to support strategic goals.
