RYZE West is seeking a dedicated LPN/RN to join our skilled nursing facility team in Chicago. You will provide total nursing care on the assigned unit, supervise staff, and ensure compliance with facility standards while supporting resident well-being.

The role emphasizes care plan development, medication administration, education of residents and families, and acting as charge nurse to support resident care decisions. A valid IL LPN or RN license is required.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN/RN license in Illinois.
• No prior abuse convictions.
• Ability to follow policies and deliver empathetic care.

Responsibilities

• Provide total nursing care to residents on the unit during the shift as an LPN/RN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments and evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and procedures.
• Monitor team performance and assist with development and corrective action as needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care and relevant information per guidelines.
• Act as a charge nurse: answer questions and support resident care decisions.
• Follow all facility safety policies and procedures.
