Job summary

ALIYA Healthcare in Palatine, IL, is seeking an LPN/RN for a PM shift to provide total nursing care on the assigned unit. You will participate in developing individualized care plans with the interdisciplinary team and administer medications with oversight for effectiveness.

Educate residents and families about care plans while ensuring safety and compliance with facility policies. You will mentor staff, delegate tasks, and remain current on infection prevention practices and nursing education.

Qualifications

• Graduate of an accredited nursing education program.
• Active Illinois LPN or RN license in good standing.
• No prior abuse convictions.
• Experience in skilled nursing/long-term care preferred.
• Strong communication and teamwork skills.
• Ability to follow facility policies and ensure resident safety.
• Ability to work with diverse resident populations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Develop and implement individualized care plans with the interdisciplinary team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, emotional, and psychological well-being.

• Recognize and respond to significant changes in resident condition.
• Accurately document resident care, treatments, and observations in records.
• Serve as Charge Nurse during the shift and support clinical decision-making.
• Follow all safety policies and regulatory requirements.
• Collaborate with physicians, therapists, and other team members to provide care.

Duties and Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN) / Registered Nurse (RN)

• Participate in developing and implementing individualized care plans alongside the interdisciplinary healthcare team

• Administer medications and treatments as prescribed and evaluate their effectiveness

• Maintain an environment that promotes residents' physical, social, emotional, and psychological well-being

• Educate residents and families regarding care plans, medications, treatments, and medical procedures

• Monitor nursing team member performance and assist with coaching, development, evaluation, and corrective action when appropriate

• Delegate nursing tasks appropriately and ensure assigned duties are completed accurately and timely

• Remain current on facility policies, procedures, infection prevention practices, and continuing nursing education

• Recognize, assess, and promptly respond to significant changes in resident condition

• Accurately document resident care, treatments, observations, and responses in accordance with facility policies and regulatory requirements

• Serve as Charge Nurse by answering questions, resolving issues, and supporting clinical decision-making during the shift

• Follow all facility safety policies, infection control standards, and regulatory requirements

• Collaborate with physicians, therapists, and other interdisciplinary team members to provide exceptional resident care

Skills and Qualifications

• Graduate of an accredited nursing education program

• Active Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing

• Previous skilled nursing, rehabilitation, or long-term care experience preferred

• Strong communication, organizational, and teamwork skills

• Ability to follow facility policies, maintain resident safety, and deliver compassionate, resident-centered care

• Ability to work effectively with diverse resident populations, including elderly and cognitively impaired individuals
