Job summary

Plum Creek Chicago is seeking a compassionate RN or LPN to join our memory care and supportive living care team. The role delivers person-centered nursing care under the Wellness Director, with a focus on assessments, medication administration, and resident comfort.

Part-time and PRN shifts run 7am–7pm, 3 days per week plus every other weekend; successful candidates hold an active IL license, and experience in memory care is preferred.

Qualifications

This role plays a vital part in delivering high-quality, person-centered care aligned with each resident’s individualized assessment and care plan — under the guidance of our Wellness Director.

Key Responsibilities

Provide daily nursing care and clinical services to residents.

Perform health assessments and monitor vital signs.

Partner with CNAs and care staff to identify resident needs and changes in condition.

Administer medications and maintain accurate documentation.

Offer clinical leadership and support to the care team.

Foster a positive, resident-focused environment.

Complete additional tasks as assigned by the Executive Director.

Qualifications

Recent experience in a Memory Care or Supportive Living environment preferred.

Requirements

Active status on the IDPH Health Care Worker Registry.

Ability to lift up to 25 lbs and perform typical nursing and CNA support duties.

Valid CPR certification (recently expired certifications accepted; recertification offered).
