Job summary

Plum Creek Chicago is seeking compassionate, dependable Certified Nursing Assistants (CNAs) to join our team. You will provide hands-on care and daily support to residents while promoting dignity, independence, comfort, and quality of life.

Qualifications

• Current CNA certification in Illinois.
• High school diploma or equivalent preferred.
• Strong communication and interpersonal skills.
• Ability to work independently while contributing to a team environment.
• Dependable, compassionate, and professional approach to resident care.
• Basic computer and documentation skills.

Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and personal care.
• Assist residents with mobility, transfers, positioning, and ambulation as needed.
• Take and record vital signs and report changes or concerns to the nursing team.
• Respond promptly to resident call lights and requests for assistance.
• Assist residents during mealtimes and with feeding as needed.
• Observe residents and communicate changes in condition, behavior, or needs to the appropriate nursing team member.
• Provide companionship, reassurance, and emotional support to residents.
• Maintain accurate and timely documentation of resident care.
• Help maintain clean, safe, and comfortable resident rooms and common areas.
• Follow established infection control, safety, and resident care procedures.
• Work collaboratively with nurses, caregivers, and other team members to provide exceptional resident care.

Day Shift: 7:00 AM – 7:00 PM
Night Shift: 7:00 PM – 7:00 AM
Schedule: 3.5 days per week | Every other weekend required

Preferred Experience

Previous experience in long-term care, skilled nursing, assisted living, supportive living, or senior care

Experience working with older adults or individuals requiring assistance with daily living
