Job summary

Fresh River Healthcare in East Windsor, CT is seeking an RN or LPN MDS Coordinator to oversee Medicare/Medicaid MDS processes and ensure accurate reimbursement. The role involves coordinating assessments and collaborating with nursing, dietary, social services, and rehabilitation.

Qualifications

• Active RN or LPN license in Connecticut.
• Knowledge of Quality Assurance/Improvement processes.
• Bachelor's or Associate degree in health care field preferred.
• Supervisory experience in long-term care preferred.
• At least 3 months experience completing MDS and training.

Responsibilities

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS)
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS.
• Ensures all current MDS/PPS updates are implemented and staff is in-serviced.
• Cross-train between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS.
• Responds to clinical emergencies, regulatory visits, surveys and issues outside of normal work hours as needed.
