Job summary

Fresh River Healthcare in East Windsor, CT is seeking an RN or LPN MDS Coordinator to oversee Medicare/Medicaid case-mix documentation and care assessments, ensuring appropriate reimbursement.

You will coordinate MDS, lead interdisciplinary care conferences, and ensure PPS/MDS updates are implemented while training staff and overseeing PPS and OBRA cross-training.

Qualifications

• Active RN or LPN license in CT.
• Knowledge of Quality Assurance/Improvement processes.
• Bachelor's or Associate degree in a health care related field (preferred).
• Supervisory experience in long-term care (preferred).
• At least 3 months experience completing MDS and PPS training or equivalent knowledge.

Responsibilities

• Coordinate MDS process through assessment, planning, implementation and evaluation.
• Lead care conferences with the interdisciplinary team.
• Complete and review Minimum Data Sets (MDS).
• Educate and train nursing staff and others on MDS roles and PPS.
• Ensure current MDS/PPS updates are implemented and staff trained.
• Cross-train between PPS and OBRA.
• Conduct regular resident interviews as required by the MDS.
• Respond to clinical emergencies, regulatory visits and surveys outside normal hours.

RN or LPN MDS Coordinator - Fresh River Healthcare- East Windsor, CT

MDS Coordinator

The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.

Responsibilities:

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS)
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS.
• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
• Will cross train between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS
• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed

Qualifications:

• Active RN or LPN license in the State of Connecticut, required.
• Knowledge of Quality Assurance/Improvement processes, required.
• Bachelors or Associate degree in a health care related field, preferred.
• Experience in a supervisory position in long-term care, preferred.
• Requires at least 3 months experience completing MDS and completion of MDS training program or equivalent experience and knowledge of PPS
