ALIYA Healthcare Consulting is seeking a dedicated LPN/RN to join a skilled nursing facility in Palos Heights, IL. The role involves providing total nursing care, supervising teammates, and ensuring compliance with facility standards.

The ideal candidate holds Illinois LPN or RN license, has strong communication and teamwork skills, and can educate residents and families while maintaining safety and high-quality resident care.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN/RN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Must be able to work with diverse resident populations, including elderly and mentally ill individuals.
• Must be physically able to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the unit during the shift as an LPN/RN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
