Job summary

Castleton Health Care Center is seeking a strong Charge Nurse / Floor Nurse (RN or LPN) to lead clinical operations on a shift-by-shift basis. The role emphasizes mentoring staff and improving resident outcomes in a caring, compliant facility.

You will supervise daily nursing activities, administer medications per orders, and serve as a clinical resource for the team while coordinating with physicians, residents, and families to maintain high standards of care.

Qualifications

• Active RN or LPN license in the state.
• Graduate of an accredited nursing program.
• 2+ years SNF experience preferred.
• Current BLS/CPR certification.
• Strong leadership, communication, and organizational skills.

Responsibilities

• Oversee daily nursing operations and ensure compliance with policies and regulations.
• Deliver and supervise high-quality resident care with a focus on positive clinical outcomes.
• Act as a clinical resource and leader for nursing staff.
• Administer medications and treatments per physician orders and facility protocols.
• Mentor, coach, and support nursing team members.
• Monitor staff performance and ensure adherence to care standards.
• Communicate effectively with physicians, residents, and families.
• Help manage staffing efficiently to reduce overtime and agency usage.
• Lead shift meetings and provide updates to the Director of Nursing.

What You’ll Do

• Oversee daily nursing operations and ensure compliance with all policies and regulations
