Job summary

RiverWoods Exeter is seeking a part-time RN or LPN (24 hours per week) to provide direct resident care in a supportive, team-focused retirement community. The role requires NH nursing licensure, a nursing degree or diploma, CPR certification, and adherence to OBRA, HIPAA, and nursing standards.

Sign-on bonus of $3,000 is offered, with competitive pay scaled by experience and excellent benefits. You will collaborate with an interdisciplinary team, formulate and update individualized care plans,

Qualifications

• Must possess a Nursing Degree or Diploma from an accredited program.
• Current NH RN or LPN license; IV administration certification preferred.
• CPR certification required; CEUs annually.

Responsibilities

• Provide direct resident care following facility policies and state regulations.
• Collaborate with interdisciplinary team to update care plans and attend care plan meetings.
• Implement falls prevention, safety and infection control protocols, and pain management measures.
• Assess and document changes in resident status and communicate with physicians and treatment team.
• Supervise and support direct care staff; delegate daily assignments effectively.
• Maintain positive relationships with residents and families to support open communication.
• Administer and document medications and treatments per nursing policies; secure narcotics.

RN/LPN Part Time 24 hours a week 7-3

RiverWoods Exeter is currently seeking a Part Time 24 hour a week day shift RN or LPN to provide direct resident care in a team-focused environment where our residents are at the heart of what we do.

Responsibilities:

• Performs professional duties in accordance with facility policies/procedures and standards of professional practice, State of New Hampshire Nurse Practice Act, OBRA and HIPAA.
• Collaborates with interdisciplinary team members to formulate individualized resident care plans. Updates care plans as necessary. Participates in Resident Care Plan Meetings.
• Institutes care protocols as indicated including: falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control guidelines (Standard Precautions), and pain assessment and management.
• Assesses and documents changes in residents’ physical or mental status and reports findings to the primary care physician and treatment team. Provides complete clinical data to facilitate decision-making.
• Provides leadership and direction for direct caregivers. Prepares daily assignments and delegates workload appropriately. Supervises direct care through observation and evaluation of direct care results.
• Establishes positive working relationships with team members, residents and family members in order to facilitate open, direct communication.
• Obtains, administers and documents medications and treatments given to residents following professional guidelines and Nursing department policies. Inventories and maintains locked system for narcotics.
• Supports, creates, and maintains a positive culture for team members and residents

Qualifications:

• Must possess, as a minimum, a Nursing Degree or Diploma from an accredited college, University or nursing school.
• Long term care facility and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.
• Must hold a registered nursing (RN, LPN) license within the State of New Hampshire. Certified in intravenous administration preferred. Must maintain CEU‘s annually. CPR is required.
