Job summary

Unifi Healthcare is seeking a Licensed Field Nurse to deliver patient-centered care across client homes. You will support independence, safety, and quality of life within individualized service plans while collaborating with the Case Manager and physicians.

Responsibilities include direct patient care, vital signs monitoring, medication administration, and care-plan collaboration, with daily travel to client homes and flexible scheduling.

Qualifications

  • Current, valid RN or LPN/LVN license
  • Reliable personal vehicle with valid insurance
  • Clear criminal background check and negative pre-employment drug screening
  • Strong verbal and written English communication skills

Responsibilities

  • Provide compassionate personal care including bathing, dressing, grooming and hygiene support
  • Assist with mobility, ambulation, wheelchair use and safe transfers
  • Support toileting needs, including catheter and ostomy care
  • Reposition bedridden clients and maintain clean bedding
  • Assist with meal preparation and nutritional intake
  • Monitor and record vital signs and document intake/output
  • Administer and manage medications as prescribed
  • Assess client nursing needs and provide nursing services per care plan
  • Report changes in condition to Case Manager and physician
  • Complete accurate clinical documentation
  • Participate in development and implementation of individualized service plans
  • Observe and report mood, behavior or emotional status
  • Communicate with interdisciplinary team
  • Perform light housekeeping to maintain a safe environment
  • Organize client living spaces to promote safety and comfort
  • Assist with errands and transportation including medication pickup and groceries

Job description

As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will play a critical role in promoting independence, ensuring safety, and enhancing quality of life for our clients and their families.

Key Responsibilities Direct Patient Care
  • Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene support
  • Assist with mobility including ambulation, wheelchair use, and safe transfers
  • Support toileting needs, including catheter and ostomy care
  • Reposition bedridden clients and maintain clean, comfortable bedding
  • Assist with meal preparation and support nutritional intake as needed
Clinical Responsibilities
  • Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature)
  • Document intake and output
  • Administer and manage medications as prescribed
  • Assess client nursing needs on an ongoing basis
  • Provide nursing services in accordance with the established care plan
  • Promptly report changes in condition, concerns, or progress to the Case Manager and physician
  • Complete accurate and timely clinical documentation
Care Plan Collaboration
  • Participate in the development and implementation of individualized service plans
  • Observe and report changes in mood, behavior, or emotional status
  • Communicate effectively with interdisciplinary team members
Supportive Services
  • Perform light housekeeping to maintain a safe, healthy environment (laundry, vacuuming, dusting, kitchen tasks)
  • Organize client living spaces to promote safety and comfort
  • Assist with errands and transportation, including medication pickup and grocery shopping
Professional Standards
  • Practice strictly within legal scope of licensure
  • Maintain professional appearance and boundaries
  • Promote teamwork, unity, and a culture of respect
  • Adapt to additional responsibilities as assigned by leadership
Working Environment
  • Daily travel to client homes and occasional visits to the agency office
  • Flexible scheduling, including potential weekends and holidays (with compensation provided)
  • Must be able to lift at least 25 pounds
  • Work environment varies based on individual client needs
Minimum Qualifications
  • Current, valid RN or LPN/LVN license
