Job summary

UNIFI Healthcare is seeking Registered Nurse/Licensed Practical Nurse to deliver comprehensive, patient-centered care to clients in their homes in the Edgewood, NM area. The role emphasizes autonomy, clinical responsibility, and building meaningful one-on-one relationships with clients and families.

As a Licensed Field Nurse, you will provide care tailored to each client’s service plan, monitor vital signs, administer medications, and collaborate with an interdisciplinary team.

Qualifications

  • Current, valid RN or LPN/LVN license.
  • Reliable personal vehicle with valid insurance.
  • Clear criminal background check and negative pre-employment drug screening.
  • Strong verbal and written English communication skills.

Responsibilities

  • Provide compassionate personal care including bathing, dressing, grooming, oral care, and hygiene support.
  • Assist with mobility including ambulation, wheelchair use, and safe transfers.
  • Support toileting needs, including catheter and ostomy care.
  • Reposition bedridden clients and maintain clean, comfortable bedding.
  • Assist with meal preparation and support nutritional intake as needed.
  • Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature).
  • Document intake and output.
  • Administer and manage medications as prescribed.
  • Assess client nursing needs on an ongoing basis.
  • Promptly report changes in condition, mood, behavior, concerns, or progress to the Case Manager and physician.
  • Complete accurate and timely clinical documentation.
  • Participate in the development and implementation of individualized service plans.
  • Communicate effectively with interdisciplinary team members.
  • Perform light housekeeping to maintain a safe, healthy environment (laundry, vacuuming, dusting, kitchen tasks).
  • Organize client living spaces to promote safety and comfort.
  • Assist with errands and transportation, including medication pickup and grocery shopping.
  • Daily travel to client homes and occasional visits to the agency office.
  • Flexible scheduling, including potential weekends and holidays (with compensation provided).
  • Must be able to lift at least 25 pounds.
  • Work environment varies based on individual client needs.
