Job summary

Unifi Healthcare seeks a Registered Nurse or Licensed Practical Nurse to provide patient-centered care in clients' homes. The role focuses on independence, safety, and quality of life, with travel to client homes and occasional agency visits.

You will follow individualized service plans and collaborate with a multidisciplinary team. Qualifications include current RN or LPN/LVN license, valid vehicle and insurance, and clear background checks.

Qualifications

• Current, valid RN or LPN/LVN license.
• Reliable personal vehicle with valid insurance.
• Clear criminal background check and negative pre-employment drug screening.

Responsibilities

• Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene support.
• Assist with mobility including ambulation, wheelchair use, and safe transfers.
• Support toileting needs, including catheter and ostomy care.
• Reposition bedridden clients and maintain clean, comfortable bedding.
• Assist with meal preparation and support nutritional intake as needed.
• Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature).
• Document intake and output.
• Administer and manage medications as prescribed.
• Assess client nursing needs on an ongoing basis.
• Provide nursing services in accordance with the established care plan.
• Promptly report changes in condition, concerns, or progress to the Case Manager and physician.
• Complete accurate and timely clinical documentation.
• Participate in the development and implementation of individualized service plans.
• Observe and report changes in mood, behavior, or emotional status.
• Communicate effectively with interdisciplinary team members.
• Perform light housekeeping to maintain a safe, healthy environment
• Organize client living spaces to promote safety and comfort
• Assist with errands and transportation, including medication pickup and grocery shopping
• Daily travel to client homes and occasional visits to the agency office
• Flexible scheduling, including potential weekends and holidays (with compensation provided)
• Must be able to lift at least 25 pounds
• Work environment varies based on individual client needs

Minimum Qualifications

• Current, valid RN or LPN/LVN license
