Qualifications

• ADN or BSN required.
• Active RN license in the state.
• Current BLS/CPR certification.
• Geriatric/long-term care experience preferred or strong clinical foundation.
• Proficient in reading, writing, and speaking English for clinical documentation.

Responsibilities

• Perform comprehensive clinical assessments and nursing diagnoses.
• Develop and adjust individualized care plans in collaboration with the interdisciplinary team.
• administer medications and treatments and monitor patient responses.
• Mentor LPNs and CNAs; delegate tasks per state regulations.
• Maintain real-time documentation in the PCC/EHR system and ensure safety.
