Qualifications

• Active RN license in New York state.
• Experience in long-term care or skilled nursing preferred.
• Strong clinical, communication, and organizational skills.
• Commitment to compassionate, resident-first care.

Responsibilities

• Deliver direct, resident-centered nursing aligned with individualized care plans.
• Coordinate with physicians, interdisciplinary team members, and families to achieve the best outcomes.
• Monitor residents’ wellbeing, assess and document changes in their conditions.
• Administer medications and treatments with safe, accurate, and efficient management.
• Promote a safe environment by maintaining infection control and regulatory standards.
• Lead with compassion by mentoring and supporting CNAs to foster a positive team culture.
