Job summary

Aura Health in Maine is seeking a Registered Nurse (RN) for a Skilled Nursing Facility focused on Long Term Care to deliver patient-centered care and coordinate with interdisciplinary teams.

You will develop, implement, and evaluate individualized care plans, educate residents and families, and supervise staff as needed in a supportive, team-driven environment. Open to all experience levels; Maine RN license required or obtainable.

Responsibilities

• Provide compassionate, high-quality patient care to residents.
• Develop, implement, and evaluate individualized care plans based on assessments.
• Educate and support patients, families, and caregivers emotionally and practically.
• Coordinate care with interdisciplinary teams, including transfers, discharges, and referrals.
• Delegate tasks to ancillary staff and perform supervisory duties as needed.
• Deliver patient-centered care, collaborate with teams, support psychosocial needs, and ensure optimal outcomes in a team-focused environment.
