The role requires an active RN license (Wisconsin DSPS) and BLS certification, with duties extending to end-of-life care, family communication, and collaboration with physicians and the care team.

Qualifications

• RN license required for Wisconsin (DSPS) and relevant education in nursing.
• BLS HealthCare Provider certification is required.

Responsibilities

• Provides skilled nursing care per established care plans.
• Guides and directs CNAs and other staff.
• Administers medications and treatments; maintains competency in nursing skills.
• Monitors resident conditions and communicates changes to physicians.
• Ensures safe medication distribution and adherence to the 5 rights.
• Oversees preventive measures and patient comfort and dignity.
• Supports end-of-life care and family communication.
• Maintains safety and adheres to regulations at all times.
• Performs other duties as assigned.
