Job summary

Careforth in Ohio is seeking an RN Care Manager or Licensed Social Worker (LSW/LISW) to onboard families, assess needs, and guide activation of services with regular home visits. You will coordinate care plans, educate caregivers, and collaborate with the clinical team to ensure quality delivery.

The role requires traveling within Montgomery/Dayton counties, a HIPAA-secure technology platform onboarding, and adherence to regulatory standards.

Qualifications

• Active RN license in Ohio or an active LSW/LISW license.
• Experience in direct elder care or disability care.
• Ability to travel within the assigned geographic area and drive an automobile.

Responsibilities

• Onboard new families and assess eligibility for services.
• Coordinate home visits and activation of services; guide through Medicaid and waiver programs.
• Educate caregivers and develop care plans; maintain ongoing coaching and support.
