Job summary

The Rector & Visitors of the University of Virginia invites applications for a Nurse Liaison to join UVA Continuum Home Health Care. You will coordinate infusion patient care, serve as the primary contact for new referrals, and collaborate with hospital and community care teams to ensure safe, effective transitions home.

Responsibilities include reviewing documentation for home infusion readiness, communicating with physicians and discharge planners, and educating patients and caregivers on IV

Qualifications

• Graduate of an accredited nursing program; BSN required within 5 years of hire.
• 1 year of relevant experience.
• Licensed RN in Virginia; VA driver's license; AHA BLS certification required.

Responsibilities

• Coordinate clinical care and ensure timely transition of infusion patients from hospital to home.
• Serve as first point of contact for new home health referrals and validate readiness for home infusion services.
• Facilitate communication among physicians, discharge planners, and interdisciplinary team members; develop individualized plans of care; educate patients and caregivers.
