Job summary

EvergreenHealth in Kirkland, WA seeks an RN Liaison to ensure smooth transitions for patients referred to Evergreen Home Care Services (EHCS), educating communities, physicians, and providers about Hospice, Palliative Care, Home Health, and Behavioral Health.

The role requires nursing qualification, BLS, 2 years home health/hospice experience, and marketing/community outreach background; the position is tied to WA state training at Kirkland hospital.

Qualifications

• Graduate of a nursing program (Diploma or ADN) required.
• Current BLS Healthcare Provider certification by date of hire.
• 2 years of Home Health, Hospice or other Home Care experience.
• Must have prior marketing, sales and/or community outreach experience.
• Current Washington State Driver's license and proof of insurability, if the position requires regular driving.
• Must have working knowledge of Medicare COP, JC Standards.
• Must be familiar with Third party insurance and Medicare HMO’s.

Responsibilities

• Grow referral base and enhance EHCS reputation as a leader of Home Care services.
• Assess appropriateness for Home Care Services across hospitals, SNFs, and other care settings.
• Interact daily with hospital, SNF, physician office, and EHCS teams.
• Maintain collaborative relationships between EHCS management and referral sources.
• Contribute to customer service measurements and data sharing with EHCS management.
• Grow relationships with referral sources across EHCS territory.
• Provide comprehensive solutions to community and stakeholders.

Job description

Wage Range: $48.21 - $87.11 per hour

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

All candidates must live in WA state and be able to come to our Kirkland hospital main campus for training/orientation

Job Summary

RN Liaison Responsible for ensuring that patients referred to Evergreen Home Care Services (EHCS) have a smooth transition to Home Care services in the care setting of their choice, educating the community, physicians and other health care providers about Hospice, Palliative Care, Home Health, and Behavioral Health services.

Responsibilities

1. Responsible for growth of referral base and consistent improvement in community reputation as a leader of Home Care services.
2. Assesses the appropriateness for Home Care Services for patients in hospitals, skilled nursing facilities, adult family homes and assisted living facilities.
3. Interacts and engages daily with hospital staff, skilled nursing facility staff, physician office staff and the Home Care administrative and clinical staff.
4. Initiates and maintains collaborative relationships and clear communication between the EHCS management and referral sources.
5. Participates in customer service measurements and functions, collecting analyzing and sharing information with EHCS management.
6. Responsible for maintaining and growing existing and new relationships with referral sources across the care continuum and throughout EHCS territory.
7. Acts as a resource to provide comprehensive solutions to the community and other stakeholders.

License, Certification, Education or Experience

REQUIRED for the position:
