Job summary
New Lebanon Rehabilitation and Healthcare Center is seeking an experienced Registered Nurse (RN) to oversee patient care, coordinate treatment plans, and ensure high-quality nursing services for residents. You will lead a multidisciplinary team, assess needs, and drive care plans in a compassionate, patient-centered environment.
The role requires an Ohio RN license and experience in long-term care or rehabilitation is preferred.
Qualifications
- Holds a valid RN license in Ohio.
- Strong clinical skills with ability to assess, plan, implement, and evaluate patient care.
- Compassionate, patient-centered leadership and communication.
- Ability to work effectively in a multidisciplinary team and build relationships.
Responsibilities
- Lead patient care: assess residents' health status, develop and implement care plans, provide direct nursing care as needed.
- Administer prescribed medications, treatments, and procedures following care protocols.
- Collaborate with physicians, LPNs, and other staff for coordinated, holistic resident care.
- Monitor health progress: evaluate conditions, document changes, communicate with care team to adjust plans.
- Promote quality care by addressing residents' physical, emotional, and social needs.
- Provide family support: communicate with families and provide updates and address questions.
