Qualifications

- Proficiency in Electronic Health Records (EHR) and digital tools.
Responsibilities
- Conduct head-to-toe assessments and initiate interventions.
- Act as the primary liaison between physicians and families.
- Manage complex medications and IV therapies.
- Provide clinical guidance and mentorship to LPNs and CNAs.
- Ensure compliance with IDPH standards and federal CMS guidelines.
