Responsibilities

• Leads the health care team with focus on patient care coordination.
• Demonstrates clinical competence and serves as a resource.
• Identifies patient needs early to develop safe care plans.
• Facilitates discharge planning and patient flow.
• Acts as charge nurse and operational leader for patient flow.
• Communicates care plans to patients and families and addresses concerns.
• Oversees staffing and orientation to department.
• Supports unit leadership with performance evaluation.
