Job Summary

Coordinates clinical activities of the Inpatient Wound Care Program, including organization, planning, implementation, evaluation, and care provision according to hospital and nursing standards.

Essential Functions
  1. Performs assessment and plans patient care.
  2. Evaluates patient assessment, planning, and intervention processes.
  3. Demonstrates professional and personal development.
  4. Delivers direct patient care appropriate to the patient's age.
  5. Assesses and diagnoses wounds, ostomies, and continence disorders.
  6. Develops and implements treatment plans.
  7. Monitors and evaluates patient progress.
  8. Provides patient education on self-care and disease management.
  9. Collaborates with healthcare professionals for holistic care.
  10. Stays updated with research and technology in wound, ostomy, and continence care.
  11. Documents patient progress and treatment plans accurately.
  12. Provides emotional support to patients and families.
  13. Manages specialized equipment and supplies.
  14. Advocates for patients' needs and rights.
  15. Manages complex wounds and continence issues.
Qualifications
Job Requirements
  • Graduate of a program in discipline; Bachelor's Degree preferred.
  • Must be able to work independently.
  • Timely wound care consults.
  • Ability to teach staff and patients about wound care, staging, and documentation.
  • Professional interaction with patients.
  • Facilitates Pressure Injury monthly meetings.
  • Accurately assesses skin integrity and prevents skin breakdown.
  • Proficient in Microsoft Office (Word, Excel, PowerPoint).
  • Basic Life Support (BLS) certification to be obtained within 6 months.
  • Current licensure by WV State Board of Nursing.
  • Wound Care Certification and 2 years of related experience preferred.
