Job SummaryCoordinates clinical activities of the Inpatient Wound Care Program, including organization, planning, implementation, evaluation, and patient care following hospital and nursing standards.
Essential Functions- Perform patient assessments and develop care plans
- Evaluate patient progress and adjust treatments accordingly
- Deliver direct patient care appropriate to patient age
- Assess and diagnose wounds, ostomies, and continence disorders
- Develop and implement treatment plans
- Monitor and evaluate patient progress
- Provide patient education on self-care and disease management
- Collaborate with healthcare professionals for holistic care
- Stay updated with research and technology in wound, ostomy, and continence care
- Document patient progress and treatment plans accurately
- Provide emotional support to patients and families
- Manage specialized equipment and supplies
- Advocate for patients’ needs and rights
- Manage complex wounds, ostomies, and continence issues
QualificationsMinimum EducationGraduate of a Program in Discipline - Required
Bachelor's Degree preferred.
Required SkillsAbility to work independently
Timely wound care consults
Ability to teach staff and patients about wound care, staging, and documentation
Professional interaction with patients
Facilitate Pressure Injury monthly meetings
Assess skin integrity accurately
Prevent skin breakdown in hospitalized patients
Assess and stage wounds accurately
Proficient in Microsoft Office (Word, Excel, PowerPoint)
CertificationsBasic Life Support (BLS) - to be obtained within 6 months
Required LicensesCurrent licensure by WV State Board of Nursing
One to two years related experience and/or training preferred
Wound Care Certification preferred
Two years of experience in wound, ostomy, and continence care required (per Wound Ostomy Continence Nursing Certification Board)
