Job summary

A multi-site outpatient mental health organization in Salisbury, Maryland is seeking a full-time IOP/BHH Nurse to provide patient care and care coordination. The ideal candidate will have an active Maryland RN license and strong documentation skills. This role involves facilitating health education for children and coordinating care for health home participants, with an emphasis on compassion and collaboration within a supportive team environment.

Qualifications

• 2+ years in behavioral health, psychiatry, or care coordination preferred.

Responsibilities

• Facilitate health education and wellness groups for children/adolescents.
• Support milieu management and collaborate with therapists and PRP staff.
• Conduct nursing assessments for enrolled Health Home participants.
• Develop and update Health Action Plans with patients and multidisciplinary teams.
• Administer medications and provide education on medication management.
