Job summary

Sheppard Pratt is seeking a Registered Nurse to serve as Assistant Director for its Health Home program, splitting time between Gaithersburg and Lanham, MD. The role focuses on integrating behavioral and physical health care to support patients’ recovery journeys and quality improvements across programs.

The ideal candidate will have strong interpersonal skills, experience in care coordination or case management, and the ability to build trust with individuals experiencing mental illness.

Qualifications

• Must be a Registered Nurse in Maryland at time of application.
• 1–3 years of nursing experience required.
• Must have car insurance and access to a vehicle.

Responsibilities

• Monitor treatment plan goals.
• Coordinate access to medical providers and hospitals, including admission and discharge.
• Ensure patients complete all required assessments, screenings, and immunizations.
• Participate in evaluation of program effectiveness and ensure continuous improvements in the quality of services.
• Assist in the occasional training of Health Home staff, specifically nurses.
• Identify quality improvement opportunities and population-level care management.
• Develop and maintain partnerships with external entities.
