Job summary

CareSource/Commonwealth Care Alliance (CCA) seeks an Integrated Care Clinical Manager in Massachusetts. This community-based RN will monitor, follow up, and manage care for dually eligible enrollees with complex medical, behavioral, and social needs.

Focus is on integrating health services with community resources to coordinate enrollee care and improve outcomes. The role includes leading an interdisciplinary team, conducting comprehensive assessments, developing care plans, and ensuring

Qualifications

RN licensed in Massachusetts with ability to independently serve dual-eligible enrollees.
Experience in care coordination or case management preferred.
Medicaid/Medicare managed care exposure preferred.
Field/community-based training is a plus.

Responsibilities

Engage enrollees in home and community settings to establish a complex care management relationship.
Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
Perform Comprehensive Assessments and functional assessments and follow up on care plans.
Coordinate with providers and community resources to ensure seamless care transitions.
Advocate for enrollees, monitor satisfaction, and ensure continuity of care.
