Job summary

Qualifications

• Active Illinois Registered Nurse license is required.
• LTC experience is preferred but not mandatory.
• Strong, effective communication with residents and team.
• Exemplary leadership and team coordination abilities.

Responsibilities

• Residents assessments.
• Care Plan development with other healthcare professionals.
• Coordination with IDT to optimize health outcomes.
• Supervision of CNAs.
• Compliance with IDPH/CMS requirements.
