Job summary

Buckeye PACE, part of One Senior Care, is seeking a Registered Nurse to provide direct and indirect nursing care for participants. You will manage complex clinical situations, develop and implement care plans, and work with an interdisciplinary team to ensure safe, effective care.

Ideal candidates are results-driven, organized, and comfortable with travel to participant homes. You will join a supportive team with ongoing training and opportunities for professional growth.

Qualifications

• Must be a graduate of an accredited nursing school.
• Current RN license within practicing state.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience with frail or elderly population is a plus; training provided.

Responsibilities

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Respond independently to emergencies and implement advanced care as needed.
• Document care, observations, and participant outcomes accurately in the electronic medical record.
• Communicate participant changes and updates clearly to the care team.
• Participate in Quality Improvement initiatives and clinical process enhancements.
• Travel to participant homes or other sites as needed to provide care.
