Job summary

Qualifications

Responsibilities

• Assess, plan, implement and evaluate patient care using the nursing process.
• Document steps of the nursing process and communicate with the care team.
• Facilitate patient discharge with referrals and education resources.
• Collaborate with other disciplines to support patient care and unit goals.
