RN - Hospital at Home (PRN)

Schedule: 1 shift weekly, 1 weekend monthly

Responsibilities

• Perform patient assessments and provide care.
• Educate patients on their condition, prescribed medications, nursing interventions, and care plans.
• Participate in the coordination of services with other disciplines and community resources as needed.
• Identify and act on changes in the patient's condition.
• Evaluate and revise care plans/assessments based on changes in patients and/or their environments.
• Document all assessments and interventions.
• Collect specimens as required (including phlebotomy).
• Administer medications including IV medications as ordered.
• Utilize and instruct patient/family on the use of the telehealth tablet used for biometric data.
• Participate in regular case conferences and in-service or special case orientations as needed and participate in on-call rotation.
• Perform other duties as assigned.

Qualifications

• Associate degree in nursing required, BSN preferred.
• Currently licensed to practice in the state in which you are required to work.
• Two years of RN experience, three to five years of inpatient RN experience in med/surg preferred.
• Experience providing RN services in the home, community, or clinic is preferred.
• BLS certification.
