Masonicare is seeking an RN Hospice Nurse for weekend Baylor in Wallingford, CT. The role focuses on delivering high-quality hospice care in the home or assisted living setting, coordinating the plan of care with other health professionals.

Qualifications

• Current RN license required.
• Hospice and/or Home Care experience preferred.

Responsibilities

• Develop, implement and evaluate the plan of care in home or assisted living settings.
• Coordinate the plan of care with other health professionals.
• Supervise LPNs and Home Health Aides; conduct supervisory visits as required.
