Job summary

Samaritan Hospice is hiring an RN to coordinate referrals and admissions, working with Nurse Managers to ensure top-tier hospice care. The role includes staffing coordination, mentoring clinical staff, and ensuring regulatory standards are met.

The ideal candidate has an NJ RN license, a BSN preferred, and 5 years medical/surgical experience; hospice or oncology background is favored. This position offers comprehensive benefits in New Jersey.

Qualifications

• BSN preferred; 5 years medical/surgical experience required
• 1 year hospice/palliative care or 3 years oncology experience preferred
• OCN, CRNH, or Board Certification in Nursing Professional Development preferred

Responsibilities

• Coordinate and implement the referral process and initiate admissions
• Collaborate with Nurse Managers to ensure optimal hospice care
• Assist manager with monthly referral center staff schedule
• Assign pending referrals and portals to staff daily
• Facilitate afternoon huddle to assess referral activity
• Promote mission values and compassionate care in daily work
• Serve as resource person, mentor and coach for clinical staff
• Manage resources to deliver quality care within cost and regulatory standards
• Handle phone calls and communications with the health care team
• Weekend obligation: one weekend per month and one holiday per year

Job description

Salary: $76,960 - $93,712.51 annually, based on years of experience.

What You'll Do:

• Responsible for coordination and implementation of the referral process and the coordination and initiation of the admissions process
• Works collaboratively with the Nurse Managers to ensure optimal hospice care
• Collaborates with Manager on complex referrals
• Assigns pending referrals to referral staff each day
• Assigns portals to specific referral staff each day
• Facilitates afternoon huddle to assess referral activity
• Accountable for ensuring mission values are translated and integrated into daily work performance and relationships being guided by the values of compassion, dignity, respect, empowerment and growth
• Utilizes excellent interpersonal skills to promote both external and internal customer satisfaction
• Functions as resource person, mentor and coach for clinical staff
• Manages resources to provide quality delivery of care within cost‑effective parameters and regulatory standards
• Manages phone calls and communications with other members of the health care team
• Assists manager with monthly schedule for referral center staff
• Weekend obligation of one (1) weekend per month as well as one (1) holiday per year
