Job summary

Samaritan is seeking an experienced RN to lead Hospice Intake in our New Jersey admissions center. This full-time role coordinates referrals and initiates admissions, working closely with Nurse Managers to ensure compassionate, compliant hospice care.

You will mentor clinical staff, allocate referrals, manage schedules, and participate in weekend/holiday coverage. The position requires NJ RN licensure, BSN preferred, and hospice/palliative or oncology experience.

Qualifications

• Valid RN license in New Jersey.
• BSN preferred; 5 years of medical/surgical experience required.
• 1 year hospice/palliative care or 3 years oncology experience preferred.
• OCN, CRNH, or Nursing Professional Development certification preferred.

Responsibilities

• Coordinate and implement the referral process and initiate admissions.
• Collaborate with Nurse Managers to ensure optimal hospice care.
• Assign pending referrals to referral staff daily.
• Assign portals to specific referral staff daily.
• Facilitate daily huddles to assess referral activity.
• Promote mission values in daily work and interactions.
• Mentor and coach clinical staff.
• Manage resources to provide quality care within regulatory standards.
• Handle communications with the health care team.
• Assist manager with monthly referral staff scheduling.
• Weekend obligation of one weekend per month and one holiday per year.

Salary: $76,960 - $93,712.51 annually, based on years of experience.

What You'll Do

• Responsible for coordination and implementation of the referral process and the coordination and initiation of the admissions process
• Works collaboratively with the Nurse Managers to ensure optimal hospice care
• Collaborates with Manager on complex referrals
• Assigns pending referrals to referral staff each day
• Assigns portals to specific referral staff each day
• Facilitates afternoon huddle to assess referral activity
• Accountable for ensuring mission values are translated and integrated into daily work performance and relationships being guided by the values of compassion, dignity, respect, empowerment and growth
• Utilizes excellent interpersonal skills to promote both external and internal customer satisfaction
• Functions as resource person, mentor and coach for clinical staff
• Manages resources to provide quality delivery of care within cost-effective parameters and regulatory standards
• Manages phone calls and communications with other members of the health care team
• Assists manager with monthly schedule for referral center staff
• Weekend obligation of one (1) weekend per month as well as one (1) holiday per year
