Job summary

Harbor Healthcare System in Jasper, TX is seeking a qualified full-time In-Patient Registered Nurse (RN) for night shifts (7:00 pm–7:00 am). The RN will admit, assess, discharge and coordinate care for patients and families as part of the interdisciplinary team.

Qualifications include Texas RN licensure and 1+ years of RN experience preferred; hospice/home health experience is a plus. Rotating weekly schedule with 12-hour shifts. Background check and drug screen required.

Responsibilities

• ADMISSIONS: coordinate admission process for patients.
• ASSESSMENTS: perform initial and ongoing patient assessments.
• DISCHARGES: coordinate discharges and care transitions.
• PRONOUNCEMENTS: handle pronouncements as needed.
• Coordinate care with interdisciplinary team.
