Job summary

Hutchinson Regional Healthcare System seeks an experienced RN for the Hospice House to develop, implement, and manage patient care plans in a compassionate clinical setting. The role emphasizes collaboration with physicians and team members to maximize patient outcomes.

The RN will participate in outstanding customer service, maintain professional relationships, and ensure accurate documentation while supporting the hospice program’s standards and goals.

Responsibilities

  • Formulates a collaborative individualized plan of care based upon medical and nursing diagnosis and desired patient outcomes for hospice patients from newborn through geriatric.
  • Assumes responsibility for the provision of skilled services in accordance with the plan of care.
  • Promotes patient symptom management and comfort in alignment with patient/family care goals.
  • Communicates patient assessment and current plan of care with other hospice staff and volunteers on a routine basis.
  • Collaborates with physician, patient/family and other assigned disciplines regarding care needs and revisions to plan of care.
  • Completes supervision of HHA services provided as per policy.
  • Completes timely and accurate documentation as per agency policy.
  • Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.
  • Observes patients’ physical, mental, emotional, and spiritual conditions and collaborates with team members as appropriate.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED :

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
  • Graduate of an accredited school of nursing.
  • Several years of clinical experience.
PREFERRED EDUCATION AND EXPERIENCE:
  • BSN.
  • Previous hospice and/or home health experience.
  • Certified as Hospice and Palliative Care Nurse.
REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
  • Current licensure as a Registered Nurse in the State of Kansas.
  • BLS - BLS is required within 3 months of hire
  • Valid State of Kansas driver's license
  • Proof of auto liability insurance in the State of Kansas
