Job summary

Grace Healthcare Services in New Jersey is seeking an RN Hospice Liaison to join our interdisciplinary team on RWJ Livingston & Jersey City campuses. This role blends clinical nursing with business development to support patients and families through transitions in care.

The RN will build referral partnerships, assess psychosocial and physical needs, coordinate comprehensive care plans, and conduct pre-discharge visits, delivering compassionate, patient-centered care in a hospital setting.

Qualifications

• Active RN license in NJ required.
• Hospice or palliative care experience preferred.
• Excellent communication and interpersonal skills necessary.
• Ability to coordinate care plans and coordinate with IDT.

Responsibilities

• Build and nurture strategic referral partnerships to facilitate referrals.
• Identify psychosocial, spiritual, and physical needs of patients in inpatient settings.
• Coordinate comprehensive, patient-centered care plans across care levels.

This role is a unique blend of clinical excellence, business development, and compassionate care, offering a fulfilling opportunity to support patients and families through critical transitions in care.

What You'll Do:

• Build and nurture strategic partnerships with new and existing referral sources to facilitate appropriate referrals.


• Identify and assess the psychosocial, spiritual, and physical needs of patients in the inpatient setting.


• Coordinate comprehensive care plans, ensuring seamless transitions to the appropriate level of care.


• Conduct pre-discharge hospital visits, offering support and guidance to patients and their families.
