Job summary

Grace Healthcare Services is seeking a Community Liaison to build strong relationships with healthcare providers, hospitals, and community organizations across Middlesex, Union, and Somerset Counties in New Jersey. The role focuses on driving referrals, educating professionals and families about hospice care, and meeting admission goals.

The ideal candidate has a Bachelor's degree or professional license (preferred), 3+ years in sales/provider relations and healthcare experience, and a valid

• Bachelor's degree or professional license preferred.
• Three+ years of sales, marketing, or provider relations experience preferred.
• Three+ years of healthcare experience preferred.
• Strong communication and relationship-building skills.
• Valid driver's license and reliable transportation required.

Responsibilities

• Develop strong relationships with healthcare providers, hospitals, and community organizations to drive referrals.
• Create and implement strategic plans to meet admission goals.
• Educate professionals and families about hospice care benefits.
• Maintain up-to-date knowledge of industry, referral sources, and competitors.
• Advocate for hospice services in the local counties.
