Qualifications

• Registered Nurse license in good standing.
• At least two years of management or supervisory experience in hospice/home care.

• Valid driver's license.
• Current CPR certification.

Responsibilities

• Receive and review case referrals and patient information.
• Assess hospice/home care needs and assign personnel.
• Confer with Medical Director regarding patient eligibility.
• Conduct clinical record reviews and communicate findings.
• Assist in employee orientation and training programs.
• Participate in quality assessment and improvement activities.
