Qualifications

• Active MO RN license required.
• Hospice or palliative care experience required.
• Demonstrated leadership in clinical teams.
• Experience facilitating interdisciplinary groups, hospice admissions, QAPI, hospice surveys, and regulatory compliance activities.
• Strong clinical judgment.
• Proficient with EMR (HCHB) and Microsoft Office.
• Ability to remain calm under pressure and multitask.

Responsibilities

• Lead and support the clinical management of hospice field staff.
• Provide day-to-day clinical guidance, oversight, coaching, and problem solving.
• Ensure safe, appropriate, and timely patient care delivery.
• Facilitate interdisciplinary group (IDG) meetings and care planning.
• Support hospice admissions and ensure a strong clinical start of care.
• Oversee clinical documentation and care coordination workflows.
• Review clinical needs and patient progression with the care team.
• Support regulatory readiness and quality monitoring.
